As a reminder, each week, your assignment will comprise a vital component of your Final Community Stakeholder Presentation and Final Community Assessment Report final projects. As such, you are encouraged to ensure that you use the feedback you receive each week to make the necessary revisions for your final report.

An action plan is a way to make sure your plan for addressing your community’s needs is concrete. It describes how you will address actionable areas supporting improving the community environment to promote health and wellness. It should be complete, clear, and current. Its development should highlight the involvement of community stakeholders, demonstrate an understanding of SMART goals, and be feasible.

You will complete the Table 5

 Download Community Health Assessment and Group Evaluation (CHANGE): Building a Foundation of Knowledge to Prioritize Community Needs[Action guide] as a guide. Complete the following:

· Using the template provided in the text, you will start by noting the project period objectives for the timelines in Step 7d Table 4. State the milestones that need to be reached. Describe the objective and why this change is a top priority for your community.

· Next, you will list an annual objective. Describe the objective, the target audience most impacted, and the estimated population size that will benefit from the change.

· Finally, you will list the activities needed to achieve the objectives. Those listed in the example are likely relevant to some of your listed community outcomes. With each activity listed, you need both a title and a brief description.

In your paper,

· Present an opening paragraph that reasonably summarizes the content that follows.

· You will complete the Table 5

Community Health Assessment and Group Evaluation (CHANGE): Building a Foundation of Knowledge to Prioritize Community Needs [Action guide] in the assigned reading as an example.

· State the two greatest needs of your community.

· Refine your Week 3 Target Audience assignment and Week 4 Barriers for Your Target Audience discussion forum efforts.

· What feedback did you receive that can help you craft a realistic action plan for addressing the top needs in your community?

· State the two target audiences that are most impacted.

· Review Action Step 7d of 
Community Health Assessment and Group Evaluation (CHANGE): Building a Foundation of Knowledge to Prioritize Community Needs [Action guide].

· Complete the 
Community Action Plan Template for the two designated issues.

· As in the sidewalk example from the 
CHANGE Action Guide (Table 4), you need to highlight the issue listed as the most significant, highlight the next steps, list your primary contacts who can make an actionable difference (list names, not titles), and provide a timeline.

· Summarize in a closing paragraph the preceding content.

· Close your paper by addressing the following for the two plans you outlined in the previous tables:

· What additional data or information are needed to help you make a more convincing argument that this is the community’s greatest need?

· What opportunities may exist for improvement within your plan?

· Where could the biggest improvements be made within your plan?

· What actions are likely to get political stakeholder support?

· What actions are likely to get community stakeholder support?

· What opportunities may exist to address health equity?

· Is the proposed action plan truly feasible? Why or why not?

· What actions best align with addressing the identified community needs, and what achievements toward your community needs are reasonable at 1, 3, 5, and 10 years?
