FINAL PAPER
Purpose: The final paper will bring together all the knowledge you gain in this course to achieve the course learning outcomes and overall purpose of the course. Each week the knowledge you gain and the information you developed for your discussion post, provide you the foundation for the final paper. The intent is that towards the end of our time together, you will have the content and information outlined for the final paper. From your content you develop in your final paper, you will develop a 5–10-minute PPT presentation to present to the interprofessional team. The presentation, provides you the opportunity to present and educate an interprofessional team, on your health-related population problem.
The overall purpose of the course addresses how evidence-driven decision-making serves to improve the health outcomes of populations and reduce inequities between population groups. We focus on examining population-level interventions that reflect an understanding of epidemiology, health policy, and social determinants of health using an ethical framework and systems thinking. We examine vulnerable populations and specific population health issues.
The final paper and presentation assignment provide students the opportunity to assess a health-related problem in a population and evaluate potential interventions for the health problem and educate an interprofessional team. Students use epidemiological data to describe the population, the health problem, the determinants for the health problem, and to identify effective interventions from research that may alleviate the health problem. Students will include health inequities, access barriers, systems level complexities, the SDOH, and the role of the advanced nurse specialist.

I.
Introduction, Problem Focus, Purpose
· Using epidemiological data, identify the health problem and describe the epidemiologic importance of the health problem in the U.S. (e.g. how it contributes to morbidity and mortality, cost of treatment, economic burden to community)
· Explain and outline the complexities (organization, structure and function) of the healthcare system and their impact on the health inequities and access barriers in this vulnerable populations
· Include how the concepts of population health thinking address the SDOH, health disparities, and healthcare outcomes
· State the Healthy People 2030 goal and objective(s) for the health problem
· State the purpose of your paper

II.
Literature Review, Interventions, Systems Thinking, Health Policy and Programs
1. Provide a clear brief description of literature search for evidenced-based interventions
2. Databases (CINAHL, OVID etc.) you searched. search terms used and rationale for search and selection decisions
3. Provide 4-5 evidence-based interventions that include screening, health promotion and disease management that you found in from your search that address the health-related problem.
4. At least two interventions must operate at the non-individual level (e.g., family, community, system, or policy level).
5. Include the systems thinking and the ethical principles that influence program and policy implementation and change.
6. Identify the ethical responsibilities of all healthcare providers to ensure interventions and changes in policy improve healthcare outcomes without causing harm or increasing disparities due to unintended consequences
7. Discuss how the healthcare system, healthcare policy and population-focused programs, impacts health equity both nationally and globally.
8. Include how you will identify, engage, and collaborate with stakeholders to implement change

III.
Health Policy and Community Focused Program Interventions
1. Develop and outline your population-focused program and framework for the implementation.
2. Detail your evaluation process, framework or tools used to monitor the effectiveness of and sustainability of the change.
3. Identify the advanced nurse leader’s role in analyzing data to improve healthcare outcomes and minimize barriers to vulnerable populations
FINAL SUBMISSION: Include all recommendations for revisions in edited final submission. Synthesize sections one, two and three to develop your final submission.
FINAL Presentation Instructions
Using the information from your paper you will develop this presentation to educate an interprofessional group of healthcare providers. The interprofessional audience are nurses, physicians, administrators, community organizations/partners, and local politicians. Develop your presentation from the knowledge and feedback from the weekly discussion questions, incorporating population health concepts, the SDOH, knowledge from your readings, and recommendations from the Future of Nursing 2020-2030. Address all the objectives for the PPT presentation.
Record the PPT presentation and upload it to the discussion board. Review the assigned peer presentations and provide constructive feedback using the Peer Review Document. Please follow the specific instructions noted on the peer-review document.
Participants (audience) in your educational session will be able to:
a. Recognize barriers faced by certain populations due to the organizational, structure and function the healthcare organizations
b. Explain the concepts of population health thinking and its impact on healthcare outcomes
c. Identify the essential role of the advanced nurse professional in analyzing data to improve healthcare outcomes and minimize barriers to vulnerable populations
d. Use systems thinking and ethical principles to identify and develop an intervention and implement a change in policy to improve healthcare outcomes and increase health equity at the national and global level.
e. Identify an evaluation plan to monitor the effectiveness and sustainability of the change.
The PPT presentation should be a minimum of 5 minutes and a maximum of 10 minutes. Please see PPT presentation rubric.
