a basic comprehension of hazards associated with a specific job,
evaluate the hazards,
research solutions
and state recommendations. The student will
communicate findings through a mix of brevity and thoroughness.

Rubric Outcomes:

· Have abilities to design and evaluate safety and health programs applicable to industrial environments

· Have the ability to identify and apply applicable standards, regulations, and codes

Outline:

·
Cover page – with student name, class name and number, and name of site

· Table of Contents

· Introductions – briefly discuss the project and why you chose the hazards listed. (1/2 page at most)

· List and briefly describe the chosen 10 hazards – 1 hazard must include a demolition hazard and 2 must include IH hazards

· Perform Risk Assessment to identify the top hazards using the provided Risk Assessment Matrix

· Use research when determining probability and severity – do not guess. You must cite your references.

· Once the top 3 hazards are identified, describe them in detail and include your companies plan to control the hazards.

· Minimum of 1 page for each

· Must have references for each (minimum of two)

· Should address regulations at a minimum

· Controls should also follow Hierarchy of controls

· Be sure to provide a bibliography listing your source(s) and provide in text citations.

·
Severe-Death or serious debilitating long-term injury such as amputation or coma

·
Serious-Permanent or nonreversible injury that severely impact enjoyment of life and may require continued treatment

·
Moderate-Permanent or reversible minor injury that does not significantly impact enjoyment of life, but requires medical treatment.

·
Slight-Reversible injury requiring simple medical treatment with no confinement
