Explain to your audience what acute care is and identify some of the health
conditions that require acute care.

Explain what range of acute care delivery models are available and
unavailable in your community:

– Create a categorized list of the acute care delivery models that are
available and unavailable in your community. Indicate the number of

each type of delivery method rather than listing individual providers for
at least the following categories:

– Hospitals; PCPs; workplace clinics; urgent care clinics; retail
care centers; telehealth options; and concierge services

Part 2: Scenario 3–5

Use the following two scenarios to illustrate accessibility of acute care
delivery models in your community at a specific time for a specific condition.

Scenario 1: In a contest for a loose ball during a pick-up soccer game, 24-
year-old Maria injured her ankle. At first, she thought it was only sprained and
went home without seeing a doctor. Now it’s after 9 p.m., her ankle is swelling
and Maria thinks she should have it examined, but she isn’t sure where to go.

Scenario 2: Over the weekend, 17-year-old Sayeed went camping. When he
returned home Sunday night, he noticed a small rash on his abdomen. He put
some anti-itch cream on it and went to sleep. By Monday morning at 9 a.m.,
the rash had tripled in size and become very uncomfortable. Sayeed and his
parents decide that he needs to seek medical attention.

For each scenario, include:

– A brief explanation or restatement of the scenario for your audience
that identifies the factors that impact the patient’s need for acute care

– A categorized list of the acute care delivery models in your community
that would be able to treat the patient’s condition

– An explanation of whether each of the delivery models included in the
categorized list would be accessible or inaccessible to the patient in
this scenario

Elaborate on the two scenarios to discuss widespread trends in patient choice
of acute care delivery methods by explaining the advantages and
disadvantages of at least three different acute care delivery models
(accessible or inaccessible) for the patients in the scenarios.

Include an analysis of gaps in acute care delivery. Propose acute care
delivery models that would fill these gaps by increasing accessibility in your
community and, thus, would be ideal for the chamber’s investment of grant
funds.
