The qualitative study by Bennetts et al. (2024) explored cardiovascular disease patients’
experiences with pharmacist-led medication management services after hospital
discharge. Using an interpretative design with semi-structured interviews, the researchers
spoke with 16 recently discharged patients from a tertiary hospital in Australia. Three
themes emerged: poor medication understanding during the transition from hospital to
home, multiple factors aPecting medication adherence such as confidence and provider
communication, and the perceived benefits of pharmacist-led postdischarge medication
reviews. Although participants recognized the value of pharmacists, their actual
engagement with these services was low. Based on the Johns Hopkins Evidence-Based
Practice model, this study is classified as Level III evidence with a High quality grade due to
strong methodology and thematic saturation.
The findings are valuable to the National Practice Problem of medication safety in primary
care and transitions of care. This study highlights patients’ real-world challenges, including
limited health literacy and gaps in communication, that can lead to medication
mismanagement. Importantly, it shows how pharmacist-led reviews can provide tailored
counseling, improve adherence, and reduce rehospitalizations. These insights emphasize
the importance of incorporating pharmacists into multidisciplinary teams, ensuring
patients receive safe, coordinated, and patient-centered medication management as they
transition from hospital to community care.
