Using Excel, design a personal health record for yourself or a family member. For this assignment, you are creating the fields and categories you would want to include in a PHR – NO health data or personal information needs to be included.
Although many vendors currently offer electronic PHRs, this assignment is taking you back to the basics of designing a PHR that will be beneficial for tracking past, present and future healthcare items. Research vendors and PHRs to decide which data elements to include on your PHR. Provide a copy of the PHR created, and a summary explaining why each of data elements included were chosen.
