1.What is “my/our professions” view of SDoH? 

As nurses and future advanced practice providers, I believe that understanding Social Determinants of Health (SDoH) is vital to our practice. These determinants significantly influence patient outcomes and overall health status. Nurses often encounter the effects of SDoH firsthand, as they play a crucial role in assessing and addressing these factors in patient care. By recognizing and addressing the social, economic, and environmental conditions that impact our patients, we can provide more holistic and effective care, ultimately improving health outcomes and promoting equity in healthcare.

2.What is “my/our professions” role in addressing SDoH? 

Up to 80% of an individual’s health is influenced by socioeconomic factors, health-related behaviors, and environmental conditions (Tiase et al., 2022).  In our profession, there are numerous ways to address SDoH; I want to keep in touch with advocacy and education.  During this week’s readings, Tiase et al. (2022) discussed screening tools used to identify social risks reflecting a person’s unmet social needs.  By ensuring we are using these tools, we are advocating for our patient’s health outcomes.  On those patients who are screened as high-risk, we need to ensure we educate them and provide any resources to help close this disparity.  As future providers the educational aspect is crucial as it can make a difference in outcomes. When our patients are educated, we are decreasing risk of non-compliance.

3.Identify 1 way each discipline would address Health Equity 

There are many healthcare disciplines in the healthcare setting, including but not limited to nurses, providers, social workers, pharmacy staff, etc. 

Nurses- Focus on individualized patient assessments that consider cultural, social, and economic factors

Providers- Implement screening protocols for social needs and prescribe medications with consideration for cost and accessibility (Tiase et al., 2022).

Social Work- Provide patients with community resources and support services

Pharmacy:  Hurley-Kim et al. (2022) discuss how helping provide medication cost alternatives and assistance programs can help minimize the barrier of financial problems—programs such as Good RX can benefit them.  Also, ensuring medication instructions are culturally and linguistically appropriate.

4.What are the factors in addressing SDoH

SDoH refers to the various conditions in which individuals are born, grow, live, work, and age, and these conditions significantly influence health outcomes. There are five significant categories of SDoH that health organizations widely acknowledge: Economic stability, education access and quality, healthcare access and quality, built environment, and community context (Centers for Disease Control and Prevention [CDC], 2021).

These five categories are interrelated and collectively impact an individual’s health and well-being (CDC, 2021). Understanding these determinants is essential for developing comprehensive public health strategies and policies to improve population health.
