Summary (nap.edu) & Crossing the Quality Chasm: A New Health System for the 21st Century Summary prompted the adoption of Electronic Health Records (EHR) to overcome medical errors reported in these articles. However, EHR- related medical errors have also been reported (Electronic Health Record–Related Events in Medical Malpractice… : Journal of Patient Safety (lww.com); EHR-related medication errors in two ICUs – PubMed (nih.gov). What are the possible causes of these medical errors and what suggestions would you make to alleviate them? Is it possible to implement the proposed changes across the USA and apply them to all brands of EHR? Why?
