Some hints for successful completion of this assignment:
You will need to help develop the data elements to be captured for THREE different data entry screens. Use your imagination to determine what screens might be needed in an ambulatory setting (registration, progress notes, discharge notes etc). In this assignment you are not creating the screen, but rather compiling the data elements to be included on each of the screens.
For each of the three screens, you must have at least EIGHT data elements that would be captured on the screen. You need to specify the purpose of the data element; specify the format the information will be entered (free text, radio buttons, check box etc); explain why the data element is important to the screen; and why the collection method is appropriate.
You can either summarize your screen elements or you could use a table format to answer the questions.
Something like this:

If you decide to use the above table format THREE tables should be submitted (one for each screen). Some of the rows might only require one word (ex: element: free text) and others will require a more lengthy explanation (Ex: purpose). Also remember since these screens are all from the same EHR, they should look similar (same color, patient name in same location etc).

Database Development
You are a HIM subject-matter expert for an information system vendor who has just entered the ambulatory electronic health record (EHR) business arena. You have been assigned the responsibility of developing data entry screens for the programmer to use in the development of the system. Your instructions are to use Microsoft Access to develop three data entry computer screens for an EHR. You must use sound user interface principles. Each form should use different types of data entry skills and should build in data quality principles. In other words, you need to use different types of entry, such as free text, radio buttons, check boxes, drop-down boxes, and the like. Because all of the screens will be part of the same EHR, they need to use the same colors, have the same look and feel, and so on. Each screen should contain at least eight data elements. All screens should have at least one drop-down box with data populated.Write narratives to describe each of the three screens that you developed. In each narrative, answer the following questions:
1. Identify the purpose of the screen.
2. Justify why each data element is important to this screen.
3. Justify why the type of data entry is appropriate for the data element.
4. Explain how you designed data quality into the screen.
