Analyze the Diagram Structure:
· Familiarize yourself with the standard cause categories presented in the fishbone diagram (i.e., People, Methods, Equipment, Environment, Policies, Procedures, etc.).

Construct Your Fishbone Diagram:
· Using the same root cause categories, construct a fishbone diagram based on your proposed healthcare improvement project.
· Populate each category with contributing factors that are relevant to the practice problem you are addressing.

Add a New Category – “Evaluation Measure”:
· Include an additional category labeled “Evaluation Measure: on your diagram.
· In this section, briefly list how you will measure the effectiveness of your proposed change (i.e., process, outcome, or balancing measures).
