1 point: Care plan is partially developed with significant gaps or errors.

0 points: Care plan is missing or does not address the patient concern adequately.

3. Evidence-based Rationale (2 points)

2 points:

Each intervention and goal is supported by a clear, evidence-based rationale, citing current research, clinical guidelines, or best practices.

1 point: Rationale is present but lacks clarity, depth, or supporting evidence.

0 points: Rationale is missing or irrelevant.

4. Organization and Presentation (2 points)

2 points:

The care plan is well-organized, neatly presented, and easy to follow.

No significant grammatical, spelling, or formatting errors are present.

1 point: Care plan is mostly organized but includes minor issues (e.g., formatting inconsistencies, small grammatical errors).

0 points: Care plan is poorly organized, difficult to read, or contains multiple errors that detract from its readability.
