Practicum Plan for Achievement of Core Clinical Objectives

1. The student will complete 30 hours (direct care) at the approved clinical site and 30 hours (indirect care) to develop a community assessment.

The preceptor and student will develop a practicum plan to achieve the core clinical objectives during the 30 hours (direct care) at the approved clinical site.

1. The practicum plan must be submitted and approved by your faculty before you begin the practicum experience. The faculty requires 48 hours to provide feedback on this plan, and you may be asked to resubmit.

Safe and Effective Care Environment

Create a safe and effective care environment for clients and health care personnel.

· Assist clinic staff during intake screenings.

· Ensure privacy and infection control protocols are followed.

· Observe and follow proper sanitation procedures.

· Monitor the clinic’s environment for safety hazards and communicate concerns to appropriate staff.

· Assist in the documentation of patient information securely and confidentially.

Health Promotion and Maintenance

Integrate knowledge of expected growth and development, health promotion and prevention strategies to achieve optimal health.

· Participate in outreach activities that align with public health priorities.

Psychosocial Integrity

Provide nursing care that promotes and supports psychosocial well-being.
