Reducing Heart failure readmissions through APRN-Led Transitional Care

The organization is short term acute care staffed beds 403, it is a level 2 trauma center. It serves
a diverse patient population, including elderly patients, those with chronic conditions, and individuals with limited healthcare access. The hospital operates with a team of cardiologists, nurse practitioners, and support staff, providing preventive and follow-up care to manage chronic heart conditions such as heart failure (HF). The facility is large, accommodating approximately 1,346 patients per
week, with a strong emphasis on patient education, medication adherence, and lifestyle modifications to prevent hospital readmission.

5. On slide 5, describe how working within a system impacts the implementation of the proposed solution, e.g., potential barriers.

6. On slide 6, identify the change theory you will use to help foster change and why you chose that particular theory.

7. On slide 7, identify your quality and safety outcomes.

8. On slide 9, identify the improvement model you will use.

9. On slide 9, discuss the financial implications for this practice change.

10. On slide 10, list the estimated key budget expenditures (does not have to be perfect).

11. On slide 11, provide the projected cost benefits or ROI.

12. On slide 12, discuss your implementation strategies.

13. On slide 13, discuss your communication plan.

14. On slide 14, discuss your evaluation plan.

15. On slide 15, provide a summary of your proposed solution and the rationale for why it is likely to make a difference.

16.  On slide 16, provide a list of 3-5 scholarly references less than 5 years old that were utilized to create this presentation. 

Use proper APA formatting when using references on the slides. Record your voiceover presentation.
