As a future nurse practitioner, it is important to understand and know which assessments are needed to make sure they are safe at home. The two assessments that can be used to determine whether a client is safe in their current living environment is conducting home safety assessment and functional assessment. Home safety assessment involves evaluating the client home for potential hazards. During the assessment, the provider will examine structure, tripping hazards, safety equipment and evaluating accessibility and mobility. The provider will also ensure the individual is safe and no physical harm to the client occurs. An individual home condition can influence ones risk of domestic violence and accident, which is why home safety is evaluated with patients and caregivers to ensure patient safety and maximize their health outcomes (Davis & Davis, 2023). Functional assessment focuses on the client ability to perform activities of daily living such as bathing, dressing, medication management and housekeeping. By assessing the client cognitive function, we can know how will the client is able to navigate through their living environment.

Discuss results that indicate the client may require additional assistance or a transition in the care environment.

There are many indicators when a client will need additional assistance or transition of care environment. As a future nurse practitioner, it is important to know when the client needs additional assistance. Some of the main factors that require clients to have additional assistance are: difficulty with mobility, depression, self-harm, complicated disorder or inability to perform activities of daily living. It is important to assess clients on how they get around at home and how they complete other task at home. If the client lacks mobility then the client is more prone to depression and careful observation is needed.

Describe the NP’s role in working with the client and family to assist with care transitions

As a future NP, I need to make sure each client has access t healthcare, access to care, safety measures and reduce emergency room visits. The NP role is to conduct medical assessments, provide health coaching and education to patients and families and provide health information and resources to client so they can receive the help they need to improve their quality of living and health.

Identify at least TWO appropriate referrals for a client who requires a transition in the care environment. How does the NP ensure coordination of care during transitions?

The two appropriate referrals for clients that require transition in the care environment is case manager and occupational therapy. The case manager will help the patient and family navigate through complicated set of services and support needed. The case manager will develop plan to optimize goals and outcomes (Giardino & De Jesus, 2023). Occupational therapy will ensure the older client is able to perform activities of daily living and will be taught to perform these activities in a safe manner to avoid any possible injuries. The NPs role during transition is to ensure there is no delays and combining treatment plan and care plan with multidisciplinary team members to ensure all-rounded care is given to the client.

Identify at least TWO public or private resources that are available for clients with limited resources in your intended practice area.

In the state of Georgia, there are many resources available for clients with limited resources. The two main non-profit organizations that will provide helpful resources is Healing Community Center and Legal Aid Atlanta. Healing Community Center serves families who have limited access to healthcare by providing family, oral, mental and women healthcare services. Legal Aid Atlanta helps low-income people meet basic needs and remove barriers to justice by providing legal aid. As a future nurse practitioner, it is important to know local and state organization that can be beneficial to your clients.
