Describe each of the three healthcare settings you identified as the proposed site for your evidence-based practice (EBP) quality improvement (QI) initiative. Be sure to address the following questions about the site: Site Selection (2 pages)

Is it a public or private entity?
Is it a stand-alone organization or a member of a larger corporation?
