After engaging with the learning resources in this module, your knowledge and understanding of the importance of “Teams” for achieving optimum outcomes has grown. 

Now it is time to examine the Team structure and Team process in your practice setting and to identify the essential Team members for promoting the success of your proposed project.

Create a presentation that provides the following information:

1.  An overview of the concept of the value of Teams in enhancing patient quality and safety, with references. (anticipated 1 to 2 slides)

2.  The structure of the Team in your practice setting.

· Construct a diagram/flow chart on a slide that depicts the Team structure in your practice setting that is relevant to your proposed student project (you are encouraged to be comprehensive in developing this aspect of the assignment)

3. Next you will construct a ‘Lean Process Map’ that delineates the current Team process relevant to the problem you have identified as the focus for graduate student project

· Create a slide that provides a brief overview of the identified ‘practice problem’ your graduate student project will be designed to address

· Create a slide that demonstrates a ‘Process Map’ that outlines the current Team process relevant to the identified practice problem [review this week’s video resource “Integrating Quality Improvement Methods to Improve Teamwork and Patient Safety (TeamSTEPPS)” minute marker 10:04 to 12:00 for a demonstration of what a process map is].

4.  Answer the following question – Do you anticipate the Team structure and processes you have identified will be a barrier to implementing your project, OR will they provide a foundation for success of implementing your proposed project? Provide a detailed rationale for your answer.

5.  Now that you have charted out and examined the existing Team structure and processes relevant to your proposed graduate student project complete the following:

· Critically examine and reflect on the initial diagram/flow chart you created earlier in the presentation for the Team structure in your practice setting. Now that you have created the ‘Process Map’ – Do you believe the Team structure you diagrammed accurately reflects the Team structure? Yes or No? Create a slide with your answer and provide an explanation that supports it.

· Construct a slide that demonstrates a ‘Process Map’ that reflects the process that you believe best supports, and is aligned with, your proposed graduate student project.
