Section 1: Discovery
1. Describe your selected nursing clinical practice problem. Explain your rationale for choosing this topic and summarize why this nursing clinical practice problem is the most important in your current or past practice area.
I chose to focus on childhood obesity because, as a pediatric and adolescent psychiatric nurse, I frequently encounter this issue within the population I serve. Obesity significantly affects both the physical and mental health of children and adolescents. In our community, childhood obesity continues to be a growing and concerning problem that needs attention and intervention.
2. Define each PICOT element.
P- (Population): Children, ages 6-12 years old.
I- (Intervention): Lifestyle change – Increased physical activity.
C- (Comparison: Sedentary lifestyle / No increase in activity.
O- (Outcome): Decrease in BMI.
T- (Time frame in months): 6 months.

4. Describe how you selected the population, intervention, outcome, and time frame.
I selected the population based on my current career path and passion, as a nurse and mother. I based the intervention, outcome, and time frame on what was measurable, practical, and effective.

The evidence directly addresses the nursing problem I have chosen, as well as the specific population. It highlights the strong link between physical activity and childhood obesity. Additionally, the evidence is current and peer-reviewed, ensuring its credibility and relevance.
3. Explain how the evidence is relevant to your selected intervention. What data from the article helps support the implementation of this intervention into practice?
The article demonstrates that an increase in physical activity has a positive impact on BMI, highlighting a direct correlation between regular physical activity and improved weight management.

It is highly relevant to the nursing practice problem of addressing obesity in the pediatric population.
4. Explain how the evidence is relevant to your selected intervention. What data from the article helps support the implementation of this intervention into practice?
The study explores using physical activity on prescription (PAP) for children with obesity, which closely aligns with the need for effective strategies to help manage and reduce childhood obesity. My interventions are increased physical activity which directly aligns with the implemented intervention in this study of using PAP.
Section 4: Evidence-Based Interventions
1. Describe the selected evidence-based nursing intervention in detail. How do you believe this intervention could be implemented in your practice area?
The evidence-based intervention I have selected is, increased physical activity vs a sedentary lifestyle to lower BMI over the period of 6 months to improve obesity in children ages 6-12 years of age. Research has demonstrated that regular physical activity plays a key role in lowering BMI and addressing childhood obesity. In my practice, this intervention could be implemented by creating individualized physical activity plans for children, promoting age-appropriate exercises, and providing ongoing support to both the children and their families.
2. Explain how the intervention supports the outcome and addresses your identified nursing clinical practice problem.
The intervention of increasing physical activity directly supports the outcome of lower BMI over a 6-month period and reducing childhood obesity by targeting one of the key factors contributing to high BMI, which is lack of physical activity.
3. What interprofessional team members would it be important to collaborate with? Explain
As a registered nurse, it would be important to collaborate with several interprofessional team members. These would include pediatricians, dietitians, physical therapists, and social workers.
This would include pediatricians due to the age of the group we are treating. Dietitians can help address the dietary needs and changes that are required in combination with the increased activity to achieve the goal of lowering BMI. Physical therapists are important to address any physical limitation the child may have and design a healthy physical activity plan that is age appropriate. The social worker can help the child and family connect with available resources in the community to improve the changes of compliance.
