Assessment 01 – Health Promotion Plan
For this assessment, plan a hypothetical clinical learning experience focused on health
promotion associated with a specific community health concern or health need. Completing this
assessment will strengthen your understanding of how to plan and negotiate individual or group
participation, and this assessment will also provide the foundation for the implementation of your
health promotion educational plan in Assessment 4.
Before you complete the detailed instructions in the courseroom, first review the specific
community health concerns or health needs below and select one to be the focus of your
assessment. Each of these health concerns has broad implications not only for the individuals
directly affected but also for the community at large in terms of healthcare costs, societal well-
being, and overall quality of life.
• Obesity: A medical condition where excess body fat accumulates to the extent that it
may have a negative effect on health.
o Potential Health Impacts: Increases the risk of heart diseases, diabetes, certain
cancers, and can lead to reduced life expectancy and quality of life.
• Diabetes: A group of diseases that result in high blood sugar.
o Potential Health Impacts: Can lead to complications like heart disease, stroke,
kidney disease, eye problems, and nerve damage.
• Drug and Alcohol Abuse: The habitual misuse of drugs and alcohol.
o Potential Health Impacts: Can lead to addiction, mental health disorders, liver
diseases, respiratory issues, cardiovascular disease, stroke, cancer, and other
debilitating conditions.
