Purpose
For this assignment, you will demonstrate your understanding of OSHA recordkeeping requirements by completing the OSHA 300 Log and 301 Incident Report using the following scenario.
Assignment Details
On April 10, 2021, at approximately 10:15 AM, Roy Reynolds, a Warehouse Associate at ABC Distribution Inc., sustained an injury while unloading a shipment of boxed electronics at Receiving Dock – Bay 3. During the unloading process, Roy attempted to manually reposition a pallet that had become misaligned on the dock lift. As he pushed the heavy pallet, it unexpectedly shifted and caused him to lose balance. In the process, his right foot was caught between the pallet and the dock plate, resulting in a contusion and a suspected fracture.
Roy wore steel-toe boots and followed standard operating procedures; however, the load weight exceeded the recommended manual handling limits, and the pallet jack was not in the immediate vicinity. He immediately reported the incident to his supervisor and was escorted to the on-site first aid station before being taken to a nearby urgent care Workers 24/7 Care for further evaluation.
Initial medical reports indicated swelling and bruising on the right foot, and X-rays were ordered to rule out a fracture. Roy was placed on restricted duty pending follow-up in 2 weeks.
