Introduction
In this task, you will act as a program coordinator at a local health department. In this role, you have been asked to write a community health report for a local health council. To do so, you will research and gather data about the social and behavioral determinants of health within a specific community of your choice. You will then analyze how various social and behavioral factors influence the health outcomes and overall well-being of the population.
Note: All references used should be current, within the last five years.

As a program coordinator for a local health department, research and analyze the public health status of a community of your choice and report your findings to the local health council by doing the following:
A. Prepare a community health report for the community by doing the following:
1. Briefly describe the community, including the following:
• the demographic characteristics of the population
• the geographic location and physical environment (e.g., urban versus rural, food sources, environmental hazards)
2. Discuss the social determinants of health, including the following:
• employment and economic stability
• education
• access to healthcare services
• housing, transportation, and neighborhood
• community context and social factors
3. Discuss the population health status and outcomes, including the following:
• maternal and child health
• chronic disease (i.e., heart disease, diabetes, cancer)
• infectious disease
4. Discuss how specific social determinants of health contribute to the following:
a. two health risk behaviors
b. two health outcome disparities
5. Discuss four barriers that contribute to community health outcomes.
Note: Types of barriers to consider include physical, social, structural, cultural, political, systemic, psychological, or environmental.
6. Design an evidence-based strategy (i.e., intervention, program, policy, initiative) to address a health risk behavior or a health outcome disparity.
a. Discuss a public health model or theory that supports the strategy.
b. Discuss how the strategy mitigates barriers to improve health outcomes.
B. Acknowledge sources, using in-text citations and references, for content that is quoted, paraphrased, or summarized.
C. Demonstrate professional communication in the content and presentation of your submission.
